By doing so, you agree fully to the terms set forth in the Journey to Peru Terms and Conditions of Travel, Liability Waiver, Statement of Purpose and Application & Registration forms, stating that you fully understand and agree to participate under the guidelines and conditions set forth in said documents ).

I understand that the TOTAL FARE OF THE TOUR IS $3450 US DOLLARS (not including international air-fare from US - Lima). To hold my space I am making a $500 non-refundable deposit via PayPal or personal check (payable to Don DeMercurio.) If your deposit is received after trip is filled or we need to cancel the trip for any reason your deposit will be returned to you immediately. Instructions for payment of deposit will follow successful submission of this form. *

I understand that an additional $1000 is due on or before March 15, 2009 (payable to Essence Institute, LLC.) *

I understand that a final payment of $1950 is due on or before May 1, 2009 (payable to Essence Institute, LLC.) *
